Order a Diagnostic Test for Pathology & Laboratory Medicine

If Children's Mercy is to complete the collection, we require either of the following:

  • A printed copy of the requisition which is given to the family to bring, or

  • A faxed copy of the requisition which may be submitted directly to the collection location.

    • We recommend, but do not require, you also provide a copy to the family when faxing.

    • Be sure to confirm the location with the family prior to submission. Faxed requisitions are only accessible at the location where it was submitted.
