A member of the Pain Management Clinic will send intake materials to patient. Upon receipt of completed intake materials, the patient will be contacted to schedule an appointment. Scheduled and requested appointment status can be found on the Wednesday referral reports. |

Follow up appointments should be scheduled by the patient by calling the Pain Management Clinic at (816) 983-6750.

Amplified pain syndromes, chronic back pain, chronic pain related to medical conditions, juvenile fibromyalgia, musculoskeletal pain, neuropathic pain
