Our goal is to support patients and families in the hospital and at home. We work closely with patients and families, nurses, social workers, physicians, home care staff, insurance providers and the community.
The Care Management team can help you:
- When you feel overwhelmed with coordinating your child’s health care needs.

- Concerns about paying for medicine and medical care.
|
- With communication between everyone caring for your child.
|
- Manage time off from your job to care for your child.
|
- Find and coordinate resources, including community resources.
|
- Concerns about your child’s development.
|
- Find out if your child qualifies for home nursing or home therapy and help with setting up these services.
|
- Get medical equipment and supplies.
|
- Get the medicine your child was prescribe.
|
- Be comfortable and get support using your child’s equipment.
|
- Get formula for your baby.
|
- Know which provider to call for your child’s needs.
|
- Get to your child’s appointments.
|
- Connect with a primary care physician.
|
- Coordinate with your child’s school, such as IEP/504.
